Preface to Fourth Edition
The Medical Review Officer's Manual, fourth edition is a reference for physicians and other providers of workplace drug and alcohol testing services. This book presents the rules and guidelines and the authors' understanding of best practices. It focuses on the review and interpretation of workplace drug and alcohol test results and related topics important to medical review officers (MROs). The standard operating procedures for MRO practice are well defined by federal regulations and guidelines, primarily those of the U.S. Department of Health and Human Services (HHS) and U.S. Department of Transportation. Many MROs follow the federal procedures when reviewing nonregulated tests because the procedures make sense, and because they are used to them. However, the federal procedures do not cover every scenario nor do they cover drugs and metabolites beyond the federal five-drug panel and tests performed on nonurine specimens. This book covers the federal procedures and other scenarios not covered by the federal procedures.
Four years have passed between this fourth edition and the third edition. This fourth edition has updates for regulatory compliance and technological changes, including changes to the HHS Mandatory Guidelines and Federal Drug Testing Custody and Control Form effective as of May 10, 2010. It also features more information about (1) nonurine tests; (2) prescription opiates and testing for prescription drugs; and (3) specimen validity testing. This fourth edition has updated endnote references. The text includes citations for some of the less well-known parts of the federal rules, so that readers can more easily locate the source material.

About the Authors
Robert Swotinsky, MD,
is chair of Fallon Clinic's Occupational Medicine Department in Massachusetts, where he leads a multisite clinical program. Dr. Swotinsky is board certified in occupational medicine, is certified as a medical review officer (MRO) and substance abuse professional (SAP), and is an aviation medical examiner. This fourth edition of The Medical Review Officer's Manual is Dr. Swotinsky's fifth book on workplace drug and alcohol testing. He is also founding editor of the MRO Update newsletter, is a coauthor of the Medical Review Officer Certification Council's (MROCC) MRO certification examination, and teaches in national courses for MROs. Dr. Swotinsky's expertise includes workplace drug and alcohol testing, toxicology, and assessment of work (dis)ability. Dr. Swotinsky graduated with chemical engineering and biology degrees from MIT, his MD degree from Vanderbilt University School of Medicine, and a Masters in Public Health from the University of California, Los Angeles.
Donna Smith, PhD,
is Regulatory Affairs and Program Development Officer at FirstLab in North Wales, Pennsylvania. Dr. Smith previously served as the Acting Director, Drug Enforcement and Program Compliance, for the U.S. Department of Transportation (DOT) in Washington, D.C. She coordinated the development, implementation, and enforcement of DOT's workplace drug and alcohol testing programs. She was a principal author of the DOT drug and alcohol testing regulations and numerous government publications on drug and alcohol testing procedures. Dr. Smith has provided testimony and statements as an expert witness on behalf of the DOT and HHS procedures for workplace drug testing. Dr. Smith has taught collectors, MROs, and other service agents in training programs throughout the United States and abroad. A graduate of Capital University of Columbus, Ohio, Dr. Smith went on to receive a Masters of Social Work from Hunter College in New York, and a doctorate in counseling psychology from Ball State University in Muncie, Indiana.
